  • Two (2) oral exams and hygiene visits per year

  • Two (2) professional cleanings per year (every 6 months)

  • One (1) complete annual diagnostic x-ray series (bitewings or full mouth; panoramic every 3–5 years)

Plan Options & Pricing

  • Single Adult: $415/year

  • Additional Family Member: $395/year

  • Periodontal Plan (includes four (4) periodontal maintenance cleanings): $675/year

​​Patients 13 years and older are eligible for the Prairie Dental Smile+ Club.

All plan fees must be paid in full at the time of enrollment or renewal. Payments for services are due at the time of treatment. No refunds will be issued for unused plans.

This is a dental discount program, not dental insurance. It cannot be combined with dental insurance, health insurance, Care Credit or other promotional offers. This plan is valid only at Prairie Dental and is nontransferable. Patients 13 years and older are eligible for the Prairie Dental Smile+ Club.
